Embarking on your journey as an unpaid carer can feel overwhelming. The challenges and responsibilities can be daunting, and knowing where to start is often the hardest part. We understand this deeply. That’s why we’ve used our extensive experience and expertise to create a clear, compassionate guide to help you navigate this path.
We are here to support you every step of the way, ensuring that no barriers stand in your way. Our goal is to help as many carers as possible, providing the guidance and support you need to embrace your role with confidence and strength.

Referral
You can choose to refer yourself or you can be referred from a variety of organisations, such as GPs, hospitals, hospices or other charities.

Triage appointment
One of our friendly referrals assistants will be in contact to go through all of the needs of the person who you care for.

Face-to-face care planning meeting
One of our experienced assessment officers will make an appointment to come to your home and build upon the information you provided in your triage appointment. They will look at your home and risk assess the environment as well as cover any key detail for your loved one’s care plan.

Care plan and risk assessment
After your face-to-face meeting you will receive a completed care plan along with risk assessments. This is your opportunity to review the documents and make sure they reflect what care and support your loved one needs.

Ready for service
When you are happy with the care plan and risk assessments you will be contact by one of our experienced service managers who will offer you a visit date and time. Your service manager will be your key contact for anything you may need.

Service starts
You will receive a letter from us outlining who will be visiting you, including the carer support worker’s name. You will also be given contact details for your service manager.

Ongoing support
The service manager will review the care and support and your documentation to ensure everything is still working and meeting your needs.
