End of
life care

& outreach

In partnership with the Surrey County Council and NHS Surrey, Crossroads provide a End of Life care support service for unpaid carers.

This is a Surrey-wide service for Carers and people affected by life limiting illnesses. They are eligible to receive tailored and safe support, at home, during their last twelve months of life.

We work collaboratively with our community partners at Surrey Heartlands, Surrey County Council and NHS Surrey to bring you resources to support Caring to the End.

Helping you to make the choices that are right for you

Most Clients prefer to stay at home, surrounded by family and friends, pets and familiar possessions. Our End of Life Carers Support Service makes it possible for the family to make the most appropriate choice suited to their situation.

We help the carer with regular breaks

A palliative care-trained Carer Support Worker will take over the caring role, giving the unpaid Carer regular breaks.

Why we’re the perfect choice

We will always put the interests of Carers and the people for whom they care above all other considerations.

We will embody professionalism with a human face.

We will always respect their individuality, dignity, choice, independence and safety.
